You can always go into creative mode and get one from there, but if you want to get it the legitimate way. Then hit a creeper a few times until it's about to die, then use a random item like a stick or a block to knock them back from you without hurting them. Then lead it in front of a skeleton. Hit the creeper into the skeleton with the stick and run towards both of them, prompting the skeleton to shoot the creeper in tha back and kill it. It's all a lot easier then it sounds.

All music discs can be obtained by getting a creeper to be killed by a skeleton's arrow. You can't craft music discs without mods.

You can't make them. They are either dropped by a creeper (rarely) or can be obtained in creative mode.
