You can use a capture card; it's simple. I use it sometimes but I don't use it for Youtube. It's a rough $100+ though, which is a bit steep…

For recording on PC you can use Bandicam. It's great for recording the game, the game audio and your mic at the same time and it has presets for YouTube - it encodes the files for you making them small and easy to upload.
